Licensure forms and documentation for BScN degree – $50
Please note the requirements to complete licensure verification documentation can be complex, depending on when the alumni graduated and the requirements of the licensure organization so allow for *4-7 weeks* to produce the completed package.
BScN alumni who are undergoing licensure verification to work in a jurisdiction outside of Ontario may require the Faculty to complete forms and provide documentation and verification of clinical placements and curriculum. Clinical and curricular breakdowns for studies that occurred prior to 2010 will be based on estimates and the limited documentation on hand. After payment, there will be a link to a page to upload the required licensure forms. If the licensure process requires course outlines in the submission, this is an additional item that needs to be ordered, as is courier delivery.

Licensure forms and documentation for NP degree – $25
Nurse Practitioner alumni who are undergoing licensure verification to work in a jurisdiction outside of Ontario may require the Faculty to complete forms and provide documentation and verification of clinical placements and curriculum. Copies of course outlines – $20
Course outlines can be produced digitally and sent via email, or via hard copy for pickup. Outlines for courses offered prior to 2010 may not be available. If this is the case a similar outline from a different offering may be used instead, and if that isn’t possible, the calendar entry will be provided. If the document needs to be sent via courier, courier delivery must be added to your order.
