| Forms Are you a graduate student looking to request a Leave of Absence (LOA)? Do you need an extension to complete coursework? Are you withdrawing from a program? There are forms for all these situations and many more. Graduate students must use the School of Graduate Studies (SGS) forms, which are availble on the SGS Student Forms page. Once you have completed your form, please return it to Student Services (fax: 416-946-0665 or in-person: 155 College Street, Reception, room 130). Once the appropriate appropriate faculty members here at Nursing have signed the form, we will forward it to SGS for processing. Please be sure to include any relevant documents with your request (i.e. medical note, etc). |
Medical Documentation If you are submitting a request or petition based on a medical concern or condition, you should include relevant medical documentation. Please download the Medical Certificate form and ask your health-care provide to complete it with you. |
