| Congratulations, you have been offered admission! In your admission package you will find a form entitled "Confirmation Form". To accept the Offer of Admission: 1) Review the information on the Confirmation Form (are the degree, start date, etc. correct?). If you have any questions about the information, email connect.nursing@utoronto.ca. 2) Tick the box "Acceptance of Admission Offer." 3) Sign and date the Confirmation Form. 4) Fax a copy of the Confirmation Form to the Bloomberg Faculty of Nursing at 416-978-8222. To confirm receipt of Confirmation Form : The Bloomberg Faculty will send you an email to confirm receipt of your Confirmation Form at the Faculty. |
