Documents Required

Applicants to the Combined Master of Nursing (Administration Field) / Master of Health Science (Health Administration) are required to complete separate applications for each program.

Please note that you will see information on supporting documents (e.g., letter of intent, various forms, etc.) that are referred to in the guidelines only after you have arranged for the application fee payment on the SGS on-line site.

Required Documents

The following forms will be available to you within the SGS on-line application. You may use the following, however, if you are having trouble accessing the forms from the application. Please ensure that you submit all documents electronically via the SGS on-line application.

i) Letter of Reference form
ii) Field of Study form
iii) Statistics Prerequisite form
iv) Completion of Clinical Hours form (NP applicants only)

Applications

On-line School of Graduate Studies Application Form

Applicants to the Master of Nursing (MN) program must complete the on-line School of Graduate Studies application form. Please click here to access the application.

Bloomberg Faculty of Nursing Entry Awards

Applicants to the Master of Nursing (MN) program who complete the SGS on-line application by January 15, 2012 are eligible to apply for Bloomberg Faculty of Nursing entry awards. To complete the entry awards application form, click here. Applicants must complete both the SGS application for admission and the Lawrence S. Bloomberg award application by January 15, 2012 to be considered for awards.
