Please note that you will see information on supporting documents (e.g., letter of intent, various forms, etc.) that are referred to in the guidelines only after you have arranged for the application fee payment on the SGS on-line site.
REQUIRED DOCUMENTS
The following forms will be available to you within the SGS on-line application. You may use the following, however, if you are having trouble accessing the forms from the application. Please ensure that you submit all documents electronically via the SGS on-line application.
i) Letter of Reference form
ii) Completion of Clinical Hours form
APPLICATION
Applicants to the Post-Master Nurse Practitioner Diploma (Post-Master NP) program must complete the on-line School of Graduate Studies application form. Please here to access the application.
