Applicants are required to complete a 2-step application process to be considered for admission. Incomplete applications will not be considered.

|
| You are required to submit your Personal Statement, Academic and Work-related References, and Prerequisite Courses and Academic Standing Assessment Form via the online FIRST IN NURSING Supplementary Application. |

|
|
|
You will be asked to respond to two questions:
i) Outline your strengths, interests and skills in 250 words; ii) Discuss your potential contribution to student life in 250 words.
You will also be asked to outline your Academic History, Leadership and Community Involvement, and Significant Accomplishments. |

|
You will be asked to provide your referees’ email addresses. Your referees will be invited to submit references electronically.
NOTE: Your academic reference should be from someone who has taught you in a university course. Your work-related reference should be from someone who has supervised you in a paid or volunteer position. |

| Prerequisite Courses & Academic Standing Assessment Form | |
| You will be asked to fill in the following information of each of your prerequisite course. You can fulfill prerequisite courses in multiple universities. If you have multiple courses that fulfill a given pre-requisite, select the courses where you were most successful. |

| Course Title |
Half or Full Course (A full course typically runs from September to April. A half course runs from September to December, or January to April. A full course is considered equivalent to 2 half courses).

It is acceptable if you have not completed all your prerequisite courses at the time of your application. However, you must fill in information of the course and the expected date of completion. If you are admitted to the program, you must fulfill all your prerequisite courses prior to registration. Please refer to Requirements for Newly Admitted Students for details.

| Academic Standing Assessment (Final Letter Grade Average) | |
|
You will be asked to report the university which granted, or will grant your degree and your academic standing (final letter grade average). You are required to obtain a minimum of mid-B (approximately 75% or 3.0 GPA) in your final year of study.
Please report the final letter grade average based on the grading scale of your university. Your final grade average will be verified and converted to the U of T grade equivalency when we receive your official transcript(s). |

You are required to submit an official transcript for each post-secondary institution you have attended by February 15, 2012. |

|
University of Toronto Transcripts
If you have an academic record at the University of Toronto since September 1998, you are not required to order a U of T Transcript. By completing your application, you agree to allow us to review your academic record available at ROSI for academic assessment.
NOTE: You are required to arrange official transcripts from the School of Continuing Studies (SCS), University of Toronto to be sent directly to the Faculty. The Faculty does not have access to records of the School of Continuing Studies. |

Transcripts from Ontario Universities
If you are attending or have previously attended an Ontario university, order your official transcripts by completing the OUAC Transcript Request Form (TRF). Please refer to OUAC for full details. |

All Other Transcripts
We require official transcript(s) to be sent directly to the Faculty from:
i) all institutions outside Ontario; ii) the School of Continuing Studies, University of Toronto; iii) the University of Toronto (Prior to September 1998) |

| English Language Facility Tests | |
|
If you are required to submit proof of English Facility, acceptable test scores must be received by the Admissions and Awards Office no later than February 15, 2012.
Late test scores will not be considered. We strongly urge applicants to write one of the tests as early as possible since test results take 4-6 weeks to reach the University. Please visit Admissions and Awards to ensure that you have met the University of Toronto’s English proficiency requirements. |

If you have an academic record at the University of Toronto since September 1998, and/or have ordered your transcript(s) via the OUAC Transcript Request Form, you can check if your transcripts have been received at Admissions and Awards Application Status Check.
Please note that we are not able to confirm receipt of your transcript(s) that are directly sent to the Bloomberg Faculty of Nursing. You will be notified via e-mail if we have not received your transcripts. Please check your email regularly. |
